PhysioLogix Physical Therapy is a boutique PT clinic led by Dr. Rob Poliszuk, DPT, offering 1:1 care for recovery, mobility, and performance. We operate from a private Miami Shores location and provide mobile in-home physical therapy for eligible patients. We accept Medicare Part B (non-homebound) with care plans up to ~30 visits based on medical necessity. For Medicare Advantage plans, we assist with gap exceptions and authorizations. Care is delivered in 3-month certification blocks for structured progress. We provide a white-glove insurance experience, handling verification, documentation, and approvals. Services include orthopedic rehab, post-surgical recovery, strength training, injury prevention, and chronic pain management.
